What is the chain-of-custody form and why is it important?

The chain-of-custody (CoC) form is a document that accompanies your specimen from your home to the laboratory, creating a legal and clinical record of who collected the specimen, when, and under what conditions. Completing the CoC form is required for your specimen to be processed at Yenos Analytical. The form includes: your full name, date of birth, OncuraKit specimen ID number (on the label of your collection cup), date and time of collection, your signature certifying that you collected the specimen and that the information is accurate. The CoC form ensures traceability — if there is ever a question about a result, the chain of custody can be reviewed. Specimens received without a completed CoC form may need to be held pending contact with the patient.
